The following picture shows a toilet flange that had been repaired by a plumber just a short time before I pulled the toilet to verify a collapsed drain line. This picture shows the flange was slanted and under the floor tile. It was also only attached to the floor with 1 bolt. Two were drilled into the flange but only one was solid. It also was not sealed so water and sewage smell leaked around the base of the toilet.

I repaired the flange properly. The cast iron flange was set 2 inches below the tile. In a case like this the spacers must be added to bring the flange up to the proper height and they must be sealed to the cast iron flange so no water or air will leak out. Below you will see the repaired flange.
